Liberty High School

Students are being asked to take part in a confidential and anonymous learning environment survey sponsored by the West Virginia Department of Education. As part of this, parents will have the opportunity to request that their student not participate in the survey through the passive consent process. These forms were distributed to students so that parents may opt them out of the survey, if they choose.  If the form is not returned, then the student will participate in the survey when it is administered. If your child did not bring a form home and you wish for them not to participate, please contact the school
