Attention Bridgeport Middle School!

The team at Healthy Harrison has developed a comprehensive approach to impacting the health disparities in West Virginia using the innovative Change Your State App. This program offers individuals the basic information to improve personal health as it relates to Mindfulness, Movement, and Nutrition. This approach has been built with research developed by nationally recognized experts and is implemented by health coaches from right here in the mountain state! Each day of the program, participants will need to accomplish their 3 goals and log in to the app to update their individual progress.

The Change Your State App will be pre-loaded to each county-issued tablet. If your device does not have the app loaded by September 24 we ask that you please notify Mrs. Jones or Mrs. Yurish so we can ensure a seamless registration process. If the app is on your device but not working properly (shows an error), we ask that you manually delete the app and redownload it from the App Store.

Creating an Account — September 24–26

All students will need to create a new account on September 24th, 25th, or 26th. FYI for returning users: We start fresh every year, so trying to use your log-in information for years past will not work.

All students will input the following information into our secure “Change Your State” App:

  1. Student Last Name

  2. Student First Name

  3. School Currently Enrolled In

  4. County or School Issued Email Address (No Personal Addresses Accepted)

  5. Password

Once participants create their new account, they will be asked to select the following:

  1. Mind Goal

  2. Move Goal

  3. Eat Goal

Kick Off — September 29

The 6-week program begins on September 29th, and students will be asked to accomplish and log their goals every day (M–F). Behavior experts tell us that new behaviors can take several weeks to become routine, so we are encouraging you to select the goals that you will be excited to do each day!

Log into Account

  • Daily Goals: Indicate completion of your daily Mind, Move, and Eat Goals (30 seconds).

  • Daily Tip: Watch videos from local experts for homework (2–3 minutes).

  • Weekly Prizes: Find out who won Change Your State Gear each week!

  • Points: Participants earn 1 point for each goal accomplished every day (3 total points per day).

  • Eligibility: Complete 15/21 goals for eligibility of weekly prize (5 days × 3 points = 15 points).

  • Prize Criteria: Prizes are awarded each Friday to selected individuals who meet the following:

    1. Logged 15 points

    2. Zero days absent

    3. Zero disciplinary referrals

  • Partner Accountability: Sometimes it is necessary to partner up to be motivated! For the weeks that we promote “Partner Accountability,” students will be selected to pick a prize and they get to bring their partner with them for a prize too! Choose a strong accountability partner to increase your chances of success!
